Palliative & End-of-Life Care Education Programs
The State of Connecticut, Department of Public Health is seeking proposals from public and private organizations and community-based agencies to provide palliative and end-of-life care educational programs to providers in various health care settings. The Request for Proposal consists of three (3) options:
Option A: Provide palliative and end-of-life care education programs for health care providers that will increase their knowledge and skills in end-of-life care.
Option B: Conduct a train-the-trainer education program on palliative and end-of-life care for providers that will increase their skills in end-of-life care at one or more of the following settings: hospitals, nursing homes, extended care facilities, prisons, home care agencies, and community-based organizations. Preference will be given to applicants who target the needs of underserved or vulnerable populations.
Option C: Provide palliative and end of life care education/certification programs for nursing and/or social work.
Applicants may apply to receive funding for one or more options. A separate and complete application is required in response to each specific option. The intent of the request is to implement strategies that will meet the goal and objectives of the Connecticut cancer Plan 2009-2013 related to palliative and hospice care: Ensure that high quality and hospice care services are available and accessible to all Connecticut residents.
