Cancer Survivorship Education Programs
The State of Connecticut, Department of Public Health is seeking proposals from public and private organizations and community-based agencies to provide cancer survivorship care educational programs for cancer care providers and survivors. The Request for Proposal consists of four (4) options:
Option A: Provide survivorship education programs that will reduce gaps and barriers in service delivery for African American/Black and separately, for Hispanic/Latino populations.
Option B: Provide an educational program that addresses the management of late/long-term effects of cancer treatment to include weight gain/loss and memory/recall for cancer survivors.
Option C: Development of a model program for survivorship care plans and coordination of follow-up care that meet the needs of Connecticut cancer survivors.
Option D: Conduct an educational conference for oncology, pediatric and primary care providers that addresses survivorship issues for pediatric cancer survivors’ transitioning to primary care.
Applicants may apply to receive funding for one or more options. A separate and complete application is required in response to each specific option. The intent of the request is to implement strategies that will meet the goal and objectives of the Connecticut cancer Plan 2009-2013 related to survivorship: To ensure a high quality of life and care for all Connecticut residents living with cancer and for their families.

A Letter of Intent must be submitted as required for this proposal by July 30, 2010.
