American Cancer Society (ACS)

Life After Treatment Survivor Guide

After Cancer Treatment is Finished is an American Cancer Society guide designed for cancer survivors transitioning out of active treatment. The booklet explains what to expect after treatment ends, including follow-up care, survivorship care plans, and treatment summaries that help patients manage ongoing health needs.

The guide also covers common long-term and late side effects of cancer treatment, emotional and social challenges survivors may experience, and healthy lifestyle recommendations to support recovery and reduce future health risks. It includes practical questions survivors can ask their healthcare team about follow-up visits, symptoms to monitor, screenings, and available support services.
